Registration

If your child meets the eligibility criteria below, fill out this form and email it to [email protected].

1. Children must be under the age of 5.
   
2. Children must reside in the following zip codes:

     65775:    West Plains school district
     65688:    West Plains school district 
     65789:    West Plains school district
     65626:    South Fork school district
     65776:    South Fork school district
     65777:    South Fork school district
     65788:    Junction Hill school district
     65790:    Fairview school district

IMPORTANT:

1. It takes 6 to 8 weeks to receive your first book.
2. Each child receives one age-appropriate book a month.
3. Parents are never asked to pay for this program. Donations are voluntary.

Donations

100% of all donations made to the Imagination Library of West Plains go toward the Imagination Library. The United Way does not deduct any admin fees. A $2 donation therefore purchases a $2 book for a child.
